Job Title:  Stroke Family Support Co-ordinator – Belfast North & West

Hours per week: 35 hours

Location:  Home-based and service venues

Salary: £ 21,589 per annum (New NJC Point 12)

Contract: Full Time Permanent

Job Description

Working closely with hospital and community stroke teams, as well as NICHS colleagues, you will make a difference to clients and their families by supporting them in achieving outcomes which will enhance their quality of life and independence.

You will be providing a needs based support service to people affected by stroke which will include:

  • Receiving referrals from a variety of sources
  • Carrying out client needs assessments
  • Pro-active referral on to NICHS programmes and other services
  • Providing listening support services and support programmes for carers

Main Responsibilities

  • To support the implementation of the NICHS Outcomes Framework and to contribute to its continuing development to meet the needs of stroke survivors and carers in the future:
  • To receive, assess and action referrals against established criteria;
  • To undertake assessments and set goals with stroke survivors using standardised tools;
  • To provide emotional support to users of the service by listening, understanding, and giving advice, information and support;
  • To liaise with health professionals and others, and play a role within multi- disciplinary teams, to ensure the assessed needs of clients and their carers are met;
  • To establish a strong network base, and refer onto other services as appropriate(both internal and external);
  • To deliver services in the community as per NICHS Service Descriptions and Standard Operating Procedures;
  • To plan and deliver all programme activity sessions in line with NICHS policies and procedures;
  • To encourage an ethos of self-management and empowerment where appropriate, to act as an advocate for the client and to ensure that client opinions are sought regularly on the services provided;
