The Transition Officer will deliver the Transition; Let’s Get Ready Service. The service fully involves young people and their families to make decisions about their life after school.
The Cedar foundation deliver a range of services that enable people with disabilities to get the most out of life and to be fully included in their communities. Our services are centred around the individual needs of each person and are delivered throughout Northern Ireland.
The Transition Officer will deliver the Transition; Let’s Get Ready Service. The service fully involves young people and their families to make decisions about their life after school. The Transition Officer will use an inter-agency approach to ensure a smooth transition post school into education, employment as well as exploring health & well-being opportunities in their local area.
Hours: 37 Hours per Week
Location: Northern Health & Social Care Trust
Office Base: The Cedar Foundation, Armour Day Centre, 6 Newal Road, Ballymoney BT53 6HD, OR 1a Woodside Road Industrial Estate, Woodside Road, Ballymena BT42 4QJ
Salary: 3 point incremental scale starting on £22,462 pro rata
