This includes supporting people to take responsibility for managing their own health and wellbeing, working in partnership with clinicians, professionals and other service providers; helping people develop the knowledge, skills and motivation to engage in civic life, work, education; to engage in interests and activities that support their health and wellbeing; and to sustain positive social and family relationships.
Working to support individuals presenting with a mental illness who may have been identified through Forensic Mental Health Services within the Northern Health and Social Care Trust (NHSCT) area. In this context the service will be directed towards individuals with a mental disorder who have been subjected to involvement of the judicial system. In addition, and working alongside the families and carers of those identified this will be done primarily through engagement with the Peer Led Relapse Prevention and Focussed Support Group model.
